Background. Non-Hodkin Lymphoma (NHL) presents one of the most common forms of hemoblastosis. The NHL morbidity index in the Republic of Moldova constitutes 4,1 to 100.000 of population. Up to date, the clearly determined diagnostic criteria of primary lienal NHL are lacking in the generalised stages. Objective of the study. Study of epidemiological data of NHL with primary damage to the spleen, depending on the morphological variant of the tumor, treatment plans. Material and Methods. 40 patients with splenic LNF diagnosis were analyzed, with evidence at the Hematological Center of the Oncological Institute of Moldova subjected to monochemotherapy, polychemotherapy, splenectomy or the combination of treatment methods. The results obtained were analysed according to the life-table method (Kaplan-Meier). Results. The average age of the group was 60 years (24 and 89 years) out of which 22 women and 18 men. In the majority the morphologicalprolymphocytic version is observed (62,5%), then the lymphoblastic follows (27,5%) and 10 % of cases – the other versions morphologic. Leukemia presented in 65% cases and 57,5% out of patients present IVB stage. Asthenia and feeling of heaviness, pain in the left hypochondrium have a share of 87.5%, 80% and 70%. The effectiveness of the combination of polychemoterapy and splenectomy treatment was identified. Conclusion. NHL with primary spleen involvement is frequently presented as a prolymphocyte morphological variant. It develops insidiously, which is why most patients find themselves in stage IVB. The most effective treatment for primary lienal NHL remains the combination of chemotherapy and splenectomy. |
