The process of reproduction inherently poses unique microbial challenges because it requires the transfer of gametes from one individual to the other, meanwhile preserving the integrity of the gametes and individuals from harmful microbes during the process. Advances in molecular biology techniques have expanded our understanding of the natural organisms living on and in our bodies, including those inhabiting the reproductive tract. Over the past two decades accumulating evidence has shown that the human microbiome is tightly related to health and disease states involving the different body systems, including the reproductive system. The aim of the study. To assess the endometrial microbiom and cytokine level in primary infertility patients. Material and methods. A prospective study was conducted, which included 96 patients divided into two groups. The study group (L1) included 48 primary infertility patients and control group (L0) – 48 fertile patients. In both groups, we performed the endometrial biopsy in the proliferative phase with the Pipelle endometrial aspiration curette. The study was approved by the Research Ethics Commission of the State University of Medicine and Pharmacy „Nicolae Testemitanu”, Chisinau, Republic of Moldova (No. 79/62 of 26.04.2017). The patients signed an informed consent to participate in the research. The statistical analysis was performed using SPSS 20 and Microsoft Excel 2016. Results. In both groups Lactobacillus spp. were detected most frequently (75% vs 79.2%, p = 0.4) followed by Enterobacteriaceae 18.8% vs 0%, p <0.001; Gardnerella vaginalis + Prevotella bivia + Porphyromonas spp., 31.3% vs 12.5%, p≤0.05 and Ureaplasma (urealiticum + parvum) 37.5% vs 6.3%, p≤0.001. The level of proinflammatory cytokines was higher in the endometrium of primary infertility patients. Conclusion. Endometrial microbiome has broad implications both in the physiology of reproduction and in its pathology. Our study has shown the presence of a pathological microbiome in primary infertility patients and its negative influence on local immunity. |
