Se prezintă cazul pacientei în vârstă de 79 de ani, la care maladia a debutat cu tuse și hemoptizii persistente, ulterior asociate cu dispnee, febră, frisoane, slăbiciune generală marcată. Cu diagnosticul de pneumonie comunitară bilaterală, anemie fierodeficitară gradul III, a primit tratamente cu antibiotice și preparate de fier, în condiții de staționar și ambulator, însă fără eficacitate. Cu agravarea stării generale a fost internată de urgență în clinica spitalului „Sf. Arh. Mihail”,. În baza analizei evoluției bolii și datelor paraclinice anterioare și efectuate în clinică s-a stabilit diagnosticul clinic de sindrom Goodpasture și revizuit tratamentul, însă fără succes din cauza duratei scurte de tratament. Modificările morfopatologice din pulmoni și rinichi au confirmat diagnosticul de sindrom Goodpasture post-mortem. Particularitatea cazului constă în depistarea unei vasculite rar întâlnite și necesitatea de a nu omite sindromul Goodpasture din diagnosticul diferențial al hemoragiilor pulmonare.

A female subject case 79 aged is presented, at which the onset of the disease was coughing and persistent haemoptysis for a long time. Subsequently associated with dyspnoea, fever, chills, and persistent lung diffuse infiltrative process. Was hospitalized with the diagnosis of previously established bilateral community-acquired pneumonia and iron deficiency anaemia grade III. No significant evolution was observed after repeated treatments with antibiotics and iron preparations. The general condition of the patient was aggravated, and she was hospitalized in our clinic. Based on the analysis of disease progression and previous clinical and paraclinical data, which was performed in other clinics, the clinical diagnosis of Goodpasture syndrome was established and the treatment has been reviewed, but without positive result. The diagnosis of Goodpasture syndrome was fixed postmortem by alterations evidenced in the kidneys and the lungs. The specificity of the case is the detection of a rare vasculitis and the importance to not omit Goodpasture syndrome of the differential diagnosis of pulmonary haemorrhage.

Представлен случай 79-летней пациентки с кашлем и постоянным кровохарканьем, впоследствии связанным с одышкой, лихорадкой, ознобом, общей слабостью. С диагнозом двусторонняя пневмония, железодефицитная анемия III степени, получалa антибиотики и препараты железа в стационарных и амбулаторных условиях, но без эффективности. С ухудшением общего состояния она была госпитализирована в больницу „Св. Aрх. Mихаил”,. На основании прогрессирования заболевания и предыдущих клинических данных был установлен диагноз синдрома Гудпасчера, и лечение было пересмотрено, но безуспешно из за несвоевременного назначения лечение. Морфопатологические изменения в легких и почках подтвердили диагноз синдрома Гудпасчера. Специфика этого случая заключается в выявлении редко встречающегося васкулита и необходимости не пропускать синдрома Гудпасчера из дифференциальной диагностики легочных кровотечений.
