Rezumat

Trombofilia reprezintă un spectru de tulburări de coagulare ale sângelui, congenitale sau dobândite, asociate cu predispoziţie pentru evenimente trombotice sau tromboembolice venoase şi/sau arteriale, care conduc la anomalii ale compoziţiei sângelui, fluxului sangvin sau peretelui vascular. Prevalenţa trombofiliilor congenitale majore în populaţia generală de Vest este estimată la aproximativ 15%, poate fi identificată de la 15% până la 25% din populațiile caucaziene şi cel puţin 50% din cazurile de TEV în timpul sarcinii sunt asociate cu o trombofilie congenitală cunoscută. Studiile epidemiologice şi experimentale au confirmat asocierea trombofiliilor ereditare şi dobândite cu TEV. Riscul TEV este cel mai mare în deficienţa de AT, deficienţa proteinei C şi deficienţa proteinei S, dar prevalenţa acestora este mică în populaţia generală. Prezenţa a doi factori trombofilici creşte de aproximativ 3 ori riscul de TEV, comparativ cu prezenţa unui singur factor. Tripla pozitivitate a AAF predispun la un pronostic mult mai rău şi contribuie la creşterea constantă a incidenţei TEV: după 2 ani acest indicator constituie 9,8%, după 10 ani - 37,1%, iar incidenţa anuală este de 5,3%.

Thrombophilia is a spectrum of congenital or acquired blood clotting disorders, associated with the predilection for thrombotic or thromboembolic venous and/or arterialeventsthat lead to disorders of blood composition, blood flow, or vascular wall. The prevalence of major congenital thrombophilias in the general Western population is estimated at approximately 15%; it can be identified in 15% to 25% of Caucasian populations and at least 50% of the cases of VTE during the pregnancy are associated with an identified congenital thrombophilia. Epidemiological and experimental studies confirmed the association of hereditary and acquiredthrombophilias with VTE. The risk of VTE is greater in AT deficiency, protein C deficiency and protein S deficiency, but their prevalence is low in the general population. The presence of two thrombophilic factors rises approximately 3 times the risk of VTE, compared to the presence of a single factor. The triple positivity of APLAs predisposes to aworse prognosis and contributes to a steady increase in the VTE incidence: after 2 years this indicator represents 9,8%, after 10 years – 37,1%, the annual incidence being 5,3%.

Тромбофилия является спектром приобретенных или наследственных нарушений свертывания крови, ассоциированных с предрасположенностью к венозным и/или артериальным тромботическим или тромбоэмболическим событиям, которые приводят к изменениям в составе крови, кровяном потоке или стенках кровеносных сосудов. Распространенность основных врожденных тромбофилий в Западном населении оценивается около 15%, они могут быть идентифицированы с 15% до 25% кавказского населения и по меньшей мере 50% случаев ВТЭ во время беременности связаны с известной врожденной тромбофилией. Эпидемиологические и экспериментальные исследования подтвердили связь наследственной и приобретенной тромбофилии с ВТЭ. Риск ВТЭ является наибольшим в дефиците АТ, дефиците белка С и дефиците белка S, но их распространенность низка у населения в целом. Наличие двух тромбофильных факторов увеличивает риск ВТЭ примерно в 3 раза по сравнению с наличием одного фактора. Тройная положительность АФА предрасполагает к значительно худшему прогнозу и способствует устойчивому увеличению частоты ВТЭ: после 2 лет этот показатель составляет 9,8%, после 10 лет - 37,1%, а годовая заболеваемость составляет 5,3%.
