Background: the SAPHO syndrome was defined as a distinct entity more than 20 years ago, its classification within rheumatologic pathologies, the mechanism and the methods of treatment remain a matter of debate. Methods and materials: The present report presents a clinical case of synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO syndrome) in an adolescent male. Results: At the onset of the disease the patient complained skin lesions since 1 year, the last 3 months associated with bone pain. Laboratory assays revealed slightly increased acute phase reactants. By X-ray and bone scintigraphy were revealed multiple bone lesions. Thus, a diagnosis of SAPHO syndrome was suspected. He responded very well to a combination therapy of nonsteroid anti-inflammatory drugs, antibiotics and retinoids. No relapse of joint pain was reported in 3 months follow up. Conclusions: The present case highlights the importance of using multiple imaging modalities to precise the diagnosis of SAPHO and indicates that early treatment of SAPHO is extremely important for a positive outcome. Despite a good prognosis, involvement of multiple organ systems could complicate the disease course. Delayed diagnosis of SAPHO can lead to continuous pain affecting the quality of life and emotional consequences caused by disfiguring skin manifestations. |
