In the study were included 1003 patients with severe preeclampsia that benefitted from obstetrical care and intensive therapy in the Women Intensive Therapy Department of IMSP and IM and C in the period from 2011 to 2015. The clinical state of the above-mentioned patients was evaluated. Results: The pregnant women were aged between 27 and 42 years old, on average 28±9,63 years old. There were 565 (56,36±6,69%) primigravidas, among which significantly prevail (p<0,05) primiparas – 64 (29±9,06%). Comparatively to 2011, in 2014 and 2015 the number of cases with severe preeclampsia practically doubled, from 148 to 247 cases. The age of pregnancy at the moment of diagnosing with severe preeclampsia was determined to decrease from 34-35 weeks in 2011 to 30-32 weeks in 2015. Significant differences in the clinical state of gestosis during these years were not manifested. From year to year a worse evolution of severe preeclamspia is determined. The rate of serious complications is higher, such as HELLP syndrome, dilated peripartum cardiomyopathy, circulatory shock with pump deficit, serious arrhythmias, ALI/ARDS, cerebral edema, reversible posterior leukoencephalopathy syndrome, cerebral vascular accidents (ischemia), acute renal failure – oliguanuric, anasarca (pleurisy, exudative pericarditis, ascites), ICE syndrome. The development of pulmonary complications in 48 patients were directly proportional to the presence of oliguanuric renal failure (Odds ratio is 4,6497; 95 % CI: 3,3423 – 6,4686, P < 0,0001). |
