The appropriate management of selection of delivering method to patients with hypertension in pregnancy have the aims of safe birth both of mother and fetus. Materials and methods: We conducted a retrospective study on patients with hypertension in pregnancy which held at the Municipal Clinical Hospital No. 1 in the period of 2009-2013. From 415 medical records of patients with hypertension in pregnancy we selected 310 cases and divided it in two groups: group I - 127 patients which born by caesarean section and the second group - 183 patients which born naturally. Results: The average age of patients is 24.3 ± 3.4 years. The history of miscarriage, premature birth and stagnation pregnancy are not associated with a signifi cant risk for cesarean section, instead patients who had the history of medical abortions have the risk to achieve cesarean intervention (OR 2.1, 95% CI 2.19 -7.15). There is not signifi cant difference of Apgar score <6 points on 5th minute among babies which were born by cesarean section 35.4% (95% CI 27.65% -44.07%), or naturally 27.9 % (95% CI 21.88% -34.77%), p>0.05. The risk of bleeding after caesarean section is 3.6 times higher than after naturally birth (OR 3.6, 95% CI 1.94 to 6.74), the risk of dehiscence suture is 2.6 times higher (OR 2.6 95 % CI 1.31 to 5.20) and the risk of fever is 1.7 times higher (OR 1.7, 95% CI 1.00 to 2.97). Conclusion. Physiological birth decrease the maternal risk , at that time the caesarean section is often associated with postpartum complications. Cesarean section effectuate on patients with hypertension in pregnancy does not improve neonatal outcomes. |
