Evaluation of postoperative outcome after thoracoscopic thymectomy in myasthenia gravis
Background. Myasthenia gravis is an autoimmune disease. Thymectomy is a therapeutic
option with benefits proved in many clinical studies. Improvement of symptoms appears
progressively during the 10-years period postoperatively. Material and methods. Postoperative outcome in 94 patients treated in our center between 2003 and 2009 was analyzed accordingly to the Clinical Research Standards proposed by the Myasthenia Gravis Foundation of America. All patents were operated using minimallyinvasive thoracoscopic approach. Results of the treatment were compared to those presented in other studies using different surgical techniques. Results. The left thoracoscopic aproach was used in 85 patients, the right-side approach – in 9 patients. Postopeative letality was zero. Complete remission was obtained in 33,5 % patients at 3 years of follow-up and in 48,3 % - at 5 years. The positive prognostic factors were age under 40 years and the short period from the onset of the disease (less then 12 months). Conclusions. Thoracoscopic thymectomy in myasthenia gravis can be performed with safety and efficacy comparable to those of open procedures. Minimally-invasive technique permits to widen the indications for thymectomy in myasthenia gravis. The optimal postoperative results can be achieved through the scrupulous selection of patients. |
