Introducere: Fistulele intestinale anastomotice, după intervenţiile adresate zonei colorectale pot conduce la complicaţii septico-purulente severe, constituind o cauză importantă a letalităţii postoperatorii. În diverse statistici incidenţa acestei complicaţii variază între 2,2%-51% Scopul lucrării: Stabilirea particularitaţilor diagnostico-curative în conduita pacienţilor cu fistule intestinale postoperatorii, apărute în rezultatul rezecţiilor de colon şi rect. Material și metode: În studiu au intrat pacienţii cu fistule intestinale postoperatorii aflaţi în clinică din 2011 până în 2018. Din 59 de pacienţi cu fistule postoperatorii, 24 fiind femei şi 35 bărbaţi. Au predominat pacienţii în vârstă de 60–70 ani ce au constituit 42%. Cauzele principale ale rezecţiilor intestinale au servit neoplaziile colorectale, 43 (72,88%) pacienţi. În dependenţă de timpul apariţiei fistulei, pacienţii au fost divizaţi în 3 grupuri. În primul grup au intrat 5 (8,47%) pacienţi cu un interval mai mic de 6 zile de apariţie a fistulei, grupul doi l-au constituit 39 (66,1%) pacienţii cu un interval de 6-10 zile şi grupul trei - 15 (25.4%) pacienţi cu un interval mai mare de 10 zile. În primului grup au necesitat relaparatomia cu drenarea şi sanarea focarului septic 3 pacienţi (60,0%), în grupul doi de relaparatomie au necesitat 8 (13,55%) pacienţi, pe când în grupul trei am recurs la relaparatomie numai într-un singur caz. Mortalitatea postoperatorie a constituit 8,47% (5) cazuri. Concluzii: Tactica curativă la pacienţii cu fistule intestinale anastamotice diferă semnificativ în dependenţă de perioada apariţiei acesteia. Cele mai grave sunt cazurile cu fistule apărute la scurt timp după intervenţia chirurgicală (< 6 zile), aceşti pacienţi vor necesita în majoritatea cazurilor relaparatomie cu sanarea focarului septic şi montarea stomei. La intervale mai mari de apariţie a fistulei, în condiţia drenării adecvate iniţial, sunt şanse de a evita relaparatomia, dar cu un monitoring minuţios, local şi general, al acestui lot de pacienţi.
Introduction: Anastomotic intestinal fistulas, that appear due to surgical interventions of colorectal region, may lead to severe septic complications, which represents an important cause of postoperative mortality. According to different sources the incidence of this complication varies between 2,2%-51%. Study purpose: is to determine the diagnostic and treatment peculiarities in patients with postoperative intestinal fistulas, appeared due to colon and rectum resections. Materials and methods: In our study were included 59 patients with postoperative intestinal fistulas that were treated in our clinic between 2011 and 2018 years, from which, 24 were females and 35 males. Patients aging between 60-70 years were predominant, which represented 42% from total. The main cause for intestinal resections were colorectal cancer, 43 (72,88%) patients. All the patients were divided into 3 groups due to time of fistula appearance after surgical intervention. First group with less than 6 days of fistula appearance constituted of 5 (8,47%) patients. Second group with 6-10 days interval constituted 39 (66,1%) patients. The third group with more than 10 day of fistula appearance constituted 15 (25,4%) patients. 3 (60%) patients from the first group had a relaparotomy for draining of the septic source, while the second group had a relaparotomy in 8 (13,55%) cases and only 1 patient from the third group needed al relaparotomy. Postoperative lethality constituted 8,47% (5 cases). Conclusions: The treatment management of patients with intestinal anastomotic fistulas significantly difference from the period of fistula appearance. The most severe cases are those fistulas that appear in less than 6 days after surgery. In most cases these patients will need a relaparotomy for drainage of septic source and usually with installation of stoma. Fistulas that appear later than 6 days, the necessity of relaparotomy decreases and in some cases can be avoided if a respected level of supervision of patient
