Background. The association between gout and kidney diseases has long been recognized. Kidney disease is very common in patients who have suffered from gouty arthritis for many years. Renal involvement is the most common manifestation of hyperuricemia after arthritis. Objective of the study. To study the clinical features and evolution of renal impairment in patients with gout. Material and Methods. Retrospective and prospective study on a group of 100 patients with gout, hospitalized in the Rheumatology and Arthrology Department of the Republican Clinical Hospital "Timofei Moșneaga", during 2018-2019. The average age of the patients was 50.64 ± 4.98 years. The average duration of the disease was 15.46 ± 1.76 years. All patients were investigated clinically and paraclinically. Results. The most common clinical manifestation of renal impairment in gout leukocyturia 45.0 ± 4.03%, followed by hematuria 39.0 ± 3.49% and albuminuria 25.0 ± 2.51%. Increased serum creatinine was found in 34.0 ± 3.27% of patients, decreased glomerular filtration in 41.0 ± 4.23% of patients, increased blood urea level in 29.0 ± 2.58% of patients, increased creatinine in urine – 17.0 ± 1.63% of patients. The incidence of the disease with an average duration of ≤10 years – 27.6 ± 2.20%, 11-20 years – 56.25 ± 5.78 %, 21-30 years – 74.2 ± 7.58%, ≥31 years – 92.46 ± 9.49%. Uric nephrolithiasis had the following incidence depending on the duration of the gout: ≤10 years – 25.46 ± 2.94%, 11-20 Conclusion. Renal impairment occurs in the early stages of gout, in 25.0 ± 2.9% of patients. The prophylaxis of kidney disease is paramount in gout, because renal dysfunction leads to decreased excretion of uric acid, hence uric acid rises in the serum, and causes more pronounced disorders. |
