Rezumat

Scopul lucrării: sunt puse în discuţie unele aspecte ce ţin de patogeneza, microbiologia, morfologia, diagnosticul clinic şi instrumental, metodele de tratament al endocarditei protezelor valvulare (EPV). Materiale şi metode: articolul prezintă o sinteză a literaturii ce cuprinde mai multe surse bibliografice de ultima oră la aceasta temă. Rezultate: datele expuse nu confirmă o diferenţă clară în frecvenţa EPV între protezele mecanice şi cele biologice. Riscul infecţiilor cauzate de proteză este cel mai mare în primele 3-6 luni postoperator, ulterior scade cu câte 0,4-1,2 % pe an. Ca agent patogen în primul an postoperator predomină S.epidermidis, 87% din acestea sunt meticilin rezistente. Ecografia transesofagiană are o sensibilitate de diagnostic al disfuncţiilor cauzate de EPV în proporţie de 82-96%. Concluzii: standardul de aur în diagnosticul EPV rămâne obţinerea culturilor sangvine şi controlul ecocardiografic în dinamică. Tratamentul chirurgical este obligatoriu în cazurile de EPV agresivă, având avantaje în comparaţie cu tratamentul conservator şi având o recurenţă mică a infecţiei în perioada de durată.

Objective: Some aspects that care about pathogenesis, microbiology, morphology, clinical diagnosis and best treatment strategies of prosthetic valve endocarditis (PVE), are put into the discussions. Methods and materials: The article presents a synthesis of the literature that holds many bibliographical resources of this subject of latter-days. Results: The exposed data do not confirm a clear difference in the frequency of PVE with mechanical and biological prosthesis. The risk of prosthetic infections is the biggest in the first postoperative 3-6 months, subsequently diminishing to 0.4-1.2% per year at late followup. As the pathogen agent in the first postoperative year with prevalence of S.epidermidis, 87% of these are meticillin resistant. The transoesophageal EcoCG has a sensibility of diagnosis caused by dysfunctions of PVE of 82-96%. Conclusion: The golden standard in diagnosis of PVE remains the obtaining of blood culture and dynamic Ecocardiografic examinations. In the aggressive course of PVE the surgical treatment is necessary, considerably diminishing the mortality comparing with conservative treatment and having a low recurrence of the infection in the long-term period.
