Th e IUGR currently constitutes one of the most complex issues of modern obstetrical and perinatal sciences, its incidence constantly increasing and infl uencing wellbeing indicators at birth. In these situations is very important to use an early diagnosis algorithm and an evidence-based decision on optimal delivery. Th e scientifi c data indicate an increase in the incidence of IUGR. Th e studies indicate diff erent risk factors in IUGR cases, as well as frequent pregnancy complications in these situations. Th ere is a need to diff erentiate between children with IUGR and children with low birth weight (under 2800 gr.) – having constitution hypotrophy as indicated by family history, paraclinical investigations and further harmonious development of these children for the longer term. Th e distinction is imperative in the light of the diff erence in obstetrical and oversight mechanisms. Th ere is a hierarchy IUGR assessment methods early in the pregnancy (28-32 weeks), most eff ective being the USG exam, followed by Doppler velocimetry that represents a complementary, however important and necessary, diagnosis means. It is also necessary to use in utero growth curbs and the percentile concept. MBased on the results of the assessment of the newborn according to the Apgar score, and the levels of the perinatal indicators in literature, the ceasarian section is the preferred delivery option in the case of IUGR. Th e long term examination of newborns with IUGR indicate that signifi cant diff erences in the development dynamics, children born by ceasarian section displaying more satisfactory perinatal results, compared to those born per vias naturalis.
