Hepatocellular carcinoma (HCC) is one of the most prevalent malignancies with increased incidence. With remarkable advances in surgical and instrumental techniques, as well as the development of molecular treatment, a large number of potentially curative treatments have become available. Management of CHC patients totally depends on the stage of the tumor and the involvement of the main vessels. Hepatic resection remains the first choice for the early stages of CHC, as it is challenged by local ablative therapy in more advanced stages. In the early stages of CHC, meeting the Milano criteria, it is possible to perform liver transplantation, which still gives good results; however, local ablation therapy may be a substitute when transplantation is not feasible. Local ablation is also used as a «bridge» therapy for liver transplantation. Recurrence of CHC is the main obstacle to successful treatment and there is currently no effective means of preventing or treating recurrence of CHC. Transarterial therapy is considered appropriate for intermediate stages of CHC, while sorafenib is recommended for advanced CHC stages. This step-based approach to therapy not only provides acceptable results but also improves the quality of life of CHC patients. Due to the complexity of CHC, therapeutic approaches need to be tailored to the characteristics of each individual patient. This review discusses current standards and trends in the treatment of CHC. |
