Surgical resection and liver transplantation are the methods that become a potentially curative in treatment in early stages. The stage of cirrhosis and liver function reserves are an integral component in the selection of optimal treatment. The purpose of this research is to determine the impact of the severity of liver cirrhosis and HCC stage to the selection of surgical treatment. 30 patients were analyzed with early and intermediate stage of HCC based on liver cirrhosis Child A, who were supposed to surgical treatment and 4 patients after transplantation. Minor hepatectomy were performed in 51% and 49% of cases – major hepatectomy. Postoperative morbidity was 38,7%, mortality – 6%. Overall 5 years survival rate was 35%, HCC recurrence occurred in 84% cases (70% local recurrence and 26% of extrahepatic manifestations of HCC). Surgical treatment of early stage of HCC based on cirrhosis, contributes to a relatively high survival rate, ensuring in the same time a better quality of life, acceptable results may be achieved in an intermediate stage of the HCC and compensated stage of cirrhosis. The presence of clinically detectable portal hypertension negatively affect post-operative results, however, it should not be taken as a contraindication for surgical resection. Liver transplant is the operation of choice in these patients with condition that they correspond to Milan criteria.. |
