Liver cirrhosis is characterized by complex changes in systemic hemodynamics. Especially renal dysfunction frequently complicates the clinical course of this disease. Hemodynamic alterations with reduced effective arterial blood volume and peripheral vasodilation are followed by activation of vasoconstrictive hormones and neurohumoral systems. According to the guidelines, acute kidney injury and chronic kidney disease are defi ned according to the serum creatinine (SCr). However, it is well known that SCr as an important indicator reflecting kidney function still has severe limitations and limited potential in the diagnosis of early kidney impairment. In predicting the outcomes of patients with cirrhosis, the Model for End-Stage Liver Disease (MELD) and MELD-Na have been shown to be superior to the traditional Child–Pugh classification and other systems of risk stratification. Doppler ultrasound measurement of the renal resistive index (RRI) is a useful method to quantify renovascular resistance in cirrhotic patients. On the basis of previous studies intrarenal RI of 0.70 was considered as a threshold value being indicative of increased renal vasoconstriction, and may even disclose progress of the liver disease before changes in laboratory results and cirrhotic patients with elevated RRIs have impaired short and long-term survival. Thus RRI may help identify high-risk patients that require special therapeutic care |
