Anatomic Study of Perforator Vessels of the Leg
Detailed knowledge regarding the vascular anatomy of the leg is of particular importance in flap surgery used for defect reconstruction. Currently,
preoperative Doppler examination is performed in order to elaborate a “map of perforators” at the level of the leg which makes the surgeon’s work easier.
However, knowing general rules regarding the predominant sites of perforator locations and more frequent intermuscular septa through which they
pass can be of real help as we know that Doppler examination can give “false positive” and “false negative” results. The aim of this anatomical study,
performed on 10 cadaveric legs, was to evaluate some anatomical particularities of perforators originate from the three main arteries of the leg in order
to find new reconstructive solutions in this anatomic area. The anatomical study emphasized that the biggest and the greatest number of perforators had
the origin from the posterior tibial artery, followed by the peroneal artery and finally by the anterior tibial artery. The latter being the most unsuitable
for perforator flap surgery as they had the smallest number of perforators per leg as well as the smallest caliber.
