Tuberculosis represents an epidemic burden for Republic of Moldova due to abbordtion of annual radiological sceening and tendence to manage TB patients in out-patient settings. For perfoming cost-effective treatment a set of patient’s targeted interventions based on risk factor assessment were identifi ed. The methodology was based on the investigation of 194 cases hospitalised in the clinical subdivisions of Pneumophysiology Institute ,,Chiril Draganiuc” in the period of 1.01.2010 – 31.12.2013. Patients were selected according to the including criteria (age >18 years old, extensive infi ltrates), and distributed in study sample 125 pulmonary TB cases and control sample 65 cases with severe pneumonia. All cases were investigated according to the national standards and immune investigations. Results: high predictability for TB shows infi ltrates in both lungs, lung destructions and dissemination, lymphocite sensibilisation to tuberculine, increased concentration of antituberculosis antibodies, decreased concentration of natural antibodies, increased IgG titre, social risk factors, harmful habits. A differentional method was developed, that showed a confidence intervel of 94,57±1,99%. A set of measures for encreasing of treatment cost-effectiveness were developed: social assistance of high risk groups, treatment for drug addiction, immune stimulating treatment |
