The following article contains a synthesis of conceptual benchmarks both from contemporary literature and individual research regarding heart failure (HF) at children having congenital heart diseases (CHD) associated with pulmonary hypertension (PH), and identifies problems for latter research. This syndrome, having the highest frequency in the structure of heart diseases at children, meets a set of pathophysiological, clinical-evolutive, prognostically and therapeutically particularities which are slightly revealed nowadays. The basic particularity of HF at pediatric age is the frequent association of LV dysfunction with RV dysfunction. The conceptual definition reveals the pathogenetical aspects implied in the evolution of HF, unanimous recognized to be similar at pediatric age (neurohumoral activation, endothelial system, vasodilatation peptides, cytokines pro-inflammatory, oxidative stress, cardiac remodeling etc.), their origin and mechanisms of actions, but nevertheless, the pathophysiological processes contributing at RV failure at children with congenital systemic-to-pulmonary shunts and CHD are not entirely known. At children with HF associated with CHD the severity predictors are not emphasized, a behavior management and an unanimously accepted treatment are missing, and there are lack of hard proofs regarding the effect of pulmonary vasodilators on cardiopulmonary hemodynamic, on cardiac remodeling processes and RV function. Therefore the studies effectuated by the author enable to appreciate the level of severity of pathogenetical modifications in the evolution of HF syndrome secondary to CHD with PH and the possibilities of corrections with the aim of improving the prognosis.). |
