Objective: To establish disability level according to the asthma evolution according to international protocol GINA. Material and method: A case-control study has been undergone, including 300 children 5 to 18 years old: 150 children with asthma and disability established and 150 children without disability established. The patients have been managed by primary doctors working in the field of asthma and re-evaluated by GINA-2009. The following criteria have been evaluated – day and night episodes, long function testing (FVC, FEV1, PEF). Results: There appeared different results while estimating the patients by primary specialists and during our study. According to our results in 22,0% patients with severe persistent asthma (p=0,0001) the diagnosed was exaggerated. Also in 10,7% patients with intermittent asthma and in 8,0% patients with light asthma the disability level has been exaggerated. That is why the study groups have been re-organized including children with sever and medium asthma. While the evaluation of the spirography indices (FEV1, PEF, FVC) did not show truthful statistical differences between study groups, (p> 0.05). Conclusions: while re-evaluating disability criteria establishment allows us to improve the management of children with asthma. |

Оценка тяжести заболевания в соответствии с критериями GINA у детей с бронхиальной астмой, имеющих инвалидность и те которые не имеют инвалидности. Материалы и методы. Для достижения данной цели было проведено исследование «случай-контроль», в которое вошли 300 детей 5-18 лет, страдающих бронхиальной астмой: 150 детей с инвалидностью по данному заболеванию и 150 – без инвалидности. Была проведена оценка степени тяжести астмы в соответствии с критериями GINA-2009 , затем полученные результаты были сопоставлены с диагнозом, установленными специалистами первичного звена медико-санитарной помощи. Анализируемые симптомы – дневные и ночные приступы, показатели функции внешнего дыхания (FVC, FEV1, PEF). Результаты. В нашем исследовании были выявлены существенные различия между оценкой специалистов и результатами переоценки. Наибольшее несоответствие наблюдалось у детей, которые имели инвалидность – специалисты преувеличили тяжесть болезни (тяжелая персистирующия БА) у 22,0% детей этой группы (р = 0,0001). Отметим, что после переоценки 8,0% детей получили инвалидность, несмотря на легкое течение астмы. В то же время появились дети с интермиттирующей астмой в обеих группах: 10,7% случаев, среди детей с инвалидностью и 13,3% – без. Затем группы были реорганизованы – все случаи тяжелой и средней тяжести БА были объединены в одну группу, как потенциальные случаи инвалидности. Но и тогда не было установлено статистических различий между средними показателями функции внешнего дыхания у детей в группах. Выводы.
