Respiratory failure is one of the most common complications in patients with acute brain injuries. Hypoxia during respiratory distress impairs the delivery of oxygen to the brain tissue and results in the conversion of aerobic metabolism from aerobic to anaerobic, which induces damage of neurocites. Chest trauma, aspiration syndrome, and cranial fractures increase the overall risk of death. Impairment of the exchange of pulmonary gases in patients with acute cerebral injuries, appreciated by using PaO2/FiO2 (arterial partial pressure of oxygen/fractional inspired oxygen content) and arterio/alveolar gradient, starts in the first 24 hours. With an early diagnosis of respiratiry failure, intubation and the start
mechanical ventilation in the prehypoxic period can stop or prevent secondary cerebral damage.
