<cfAbstr cfLangCode='RO' cfTrans='o'>Hidrocalicoza reprezintă dilataţie sau distensie a calicelor renale, cel mai adesea asociată cu hidronefroza; se pune în evidenţă prin ecografie sau urografie intravenoasă. Hidronefroza
congenitală este determinată de obstrucţia joncţiunii pielo-ureterale ce produce distensia bazinetului şi calicelor şi leziuni atrofice ischemice ale parenchimului renal. Este cea mai întâlnită anomalie a ureterului, fiind mai frecventă de aproape 3 ori la băieţei în raport cu fetiţele. Artrita reactivă se defineşte ca fiind artrita care apare la 1-4 săptămâni după o infecţie enterală sau urogenitală, mai ales la persoanele cu HLA B27. Se prezintă un caz clinic din secţia reumatologie a IMSP Institutul Mamei şi Copilului. Un băieţel cu vârsta de 8 ani prezenta următoarele acuze: artralgii, tumefiere, limitarea mişcărilor şi hipertermie locală la nivelul articulaţiilor talocrurale bilateral şi interfalangiene ale ambelor mâini, mers şchiopătat, pumni incompleţi, fatigabilitate, slăbiciune generală, apetit scăzut. La copil s-a efectuat ultrasonografia organelor renale şi urografia intravenoasă, unde s-au determinat funcţia renală încetinită pe dreapta la 6 şi 20 min. Calicile rinichiului drept apar dilatate marcat ≈20mm, ureterul drept nu se contrastează. În ortostatism - marcat dilatate calicile rinichiului drept ≈30mm cu nivele de contrast. Astfel, a fost stabilit diagnosticul de hidrocaliconefroză pe dreapta.</cfAbstr>
<cfAbstr cfLangCode='EN' cfTrans='o'>Hydrocalicosis is a dilatation or distension of the renal calyces, often associated with hydronephrosis, it is revealed by ultrasound or intravenous urography. Congenital hydronephrosis is caused by the obstruction of the pelvis-ureteral junction that produces distention of the pelvis, calyces and renal parenchymal ischemic atrophic lesions. It is the most common abnormality of the ureter, being almost 3 times more common in boys compared with girls. Reactive arthritis is defined as arthritis occurring in 1-4 weeks after the enteric or urogenital infection, especially in individuals with HLA B27.
This article presents a clinical case from the Rheumatology Department of the Institute of Mother and Child. An 8 year old boy had the following complaints: arthralgia, swelling, limitation of movement and local hyperthermia in bilateral talocrural and interphalangeal joints of both hands, limping, incomplete fists, fatigue, general malaise, poor appetite.
The child underwent a renal organs ultrasonography and intravenous urography, where it was determined that the right renal function was slowed to 6 and 20 min. The right kidney calyces are considerably marked ≈ 20mm, the ureter is not shown. In standing position – the calyces of the right kidney were also considerably dilated ≈ 30mm with contrast levels. Thus, the right hydrocaliconephrosis was diagnosed.</cfAbstr>
