Rezumat

This article is an overview of the most important etiopathogenic, diagnostic and terapeutical aspects of acne vulgaris (AV). It shows the new developements in the contemporary treatment of acne. The remedies of choice are cyclins and macrolides, antiandrogens, systemic and topical retinoids, micronutrients, etc. Particularly important is specific and nonspecific immunomodulatory treatment. Azelaic acid, boswelic acid, glycyrrhizinic acid, naphthoic acid, zinc acetate and zinc hyaluronate are widely used. Magnito-laser therapy and/or dermatological surgery are/is recommended in severe forms of inflammatory lesions and/or postinflammatory scars. Recently, antiandrogens, especially, spironolactone, have returned to be used. Spironolactone competitively inhibits testosterone of adrenal and gonadal origin by reducing 17 α-hydroxylase, it also antagonizes dihydrotestosterone influence on the peripheral receptors. Spironolactone is a potassium sparing diuretic, what leads to the appearance of such side effects as hyperkalemia. A small dose of furosemide is beneficial to be added to the complex therapy for the electrolyte imbalance (hiperkaliemia) prophylaxis. As a result there is a reduction of dermal edema on the one hand and a stimulation of the detoxification processes – on the other hand. Semilact, a combination of 100 mg spironolactone and 20 mg furosemide, is the optimal choice. Postmarketing study carried out on the 30 women with various forms of AV have revealed good and very good results in 87% of patients. Semilact can be a successful alternative treatment of the patients with AV.
