The aim of the study is to analyse the diagnostic accuracy of physical examination and color Doppler sonographic examination in the preoperative distinction of inguinal hernias. 368 consecutive patients with inguinal hernia recieved a clinical examination and preoperative diagnosis of the type of hernia. The coughing test was used to distinguish between direct, indirect and femoral hernia. In 277 cases the color Doppler ultrasound had been
used to asist with case management. The criteria used for the diagnosis of inguinal hernia by ultrasound consisted of the finding of the hernial sac or a defect of the fascia transversalis on Valsalva maneuver. On color duplex exam the relationship between the inferior epigastric artery and hernal sac were
examined. The data obtained were compared to the intra-operative findings. The accuracy of the physical examination for direct inguinal hernia was 88% for patients with palpable groin mass and 81% for non-palpable hernias. The accuracy of sonographic examination for direct inguinal hernia amounted 100% (palpable hernias) and 95% (non-palpable hernias). Conclusions: the ultrasound color duplex examination is a valuable tool in the preoperative non-invasive differentiation of inguinal hernias. The combined physical and sonographic exams may help classify groin hernias with great accuracy. The preoperative distinction of the hernia type helps to select the proper surgical procedure. |
