The aim of the study was to compare the clinical efficacy and safety of isotonic and hypertonic saline nasal irrigation in combination with vasoconstrictors and hypertonic saline irrigation without vasoconstrictors. Material and Methods: In order to achieve the objectives of the study, 45 patients ages 22-40 years, with acute or chronic (in exacerbation) rhinosinusitis were selected. Patients were divided into 3 groups. Group I consisted of
15 patients who in addition to conventional medical therapy (anti-bacterial, local vasoconstrictors, antihistamines, mucolytics) received isotonic saline irrigation (“Tonimer Normal”, Istituto Ganassini, Italy). Group II consisted of 15 patients who in addition to conventional medical therapy received hypertonic saline irrigation (“Tonimer Hypertonic”). Group III consisted of 15 patients who, in addition to the conventional therapy received hypertonic saline irrigation (“Tonimer Hypertonic”) without the use of local vasoconstrictors. Dosage and method of application of a series of Tonimer match official recommendations: washing the nose 2-3 times a day, duration of treatment – 7 days. Results: Group I – objective symptoms and endoscopic picture improved moderately. In Group II and Group III – greater improvement in symptoms and endoscopic data was observed. Results in patients of group II and group III (without the use of decongestants) were identical. Conclusions: nasal irrigation using hypertonic saline solution is effective and safe in the treatment of acute and chronic (in exacerbation) rhinosinusitis; improves nasal breathing, reduces swelling of the nasal mucosa, facilitates evacuation of nasal secretion, well tolerated (no side effects were noted), allows to reduce treatment time and to avoid complications caused by topical nasal decongestants, since they do not act systemically on the body, no interaction with other drugs, and, for this reason, can be recommended as complementary therapy of rhinitis, especially in patients for whom decongestants are contraindicated (pregnant women, patients with arterial hypertension). |
