Our purpose was to determine the justification of the cervical cerclage for pacients with multiple pregnancy and sonographic shot cervix. To achieve this goal we analyzed all multiple pregnancies with cervical cerclaje that took place in the Municipal Clinical Hospital No. 1 during 2008-2010. In this study a group with singleton pregnancies was formed. We established the incidence of cervico-istmical insufficiency in multiple pregnancy and the evolutions of this pregnancies and perinatal outcomes. We established that:1. The cervical cerclage is justified in multiple asymptomatic pregnancies in which ultrasonographically a shortening of the uterine cervix equal or less than 25mm is determined. In case of singleton pregnancy this procedure is applied only in 15mm or less length of the cervix in an associations with miscarriage and preterm birth anamnesis.2. The application of the cervical cerclage in multiple pregnancy avoid preterm births between 22- 30 weeks and so the children’s with extremely low mass. Nor a multiple pregnancy in this group of study was finished less than 30 weeks.3. All women with a complicated anamnesis due to late miscarriage or/and preterm births require a strict transvaginal ultrasonography evaluation between 14-24 weeks of gestation. In multiple pregnancy this examination mast be done no matter what anamnesis the patient has.4. Istmico-cervical insufficiency in pregnancy is approximateve 5 time higher in twin pregnancy in comparison with singletons. |
