Rezumat

Целью двойного “слепого”, плацебоконтролируемого клинического испытания было уточнение роли антилейкотриеновых препаратов в менеджменте бронхиальной астмы у детей и разработка оптимальной терапевтической тактики. Задачи исследования состояли в определении аддитивного и гормонсберегающего эффекта модификатора лейкотриенов Монтелукаст (Зеспира®) у детей с персистирующей бронхиальной астмой среднетяжелого течения, оценке клинической эффективности препарата при атопическом синдроме. В ходе исследования было доказано, что после 12 недель лечения у детей, принимавших препарат Зеспира, в 3 раза уменьшилась частота астматических приступов (p<0,01) и снизилась потребность в β2-агонистах (p<0,01), что позволило двукратно снизить дозу ингаляторных стероидов без ущерба для здоровья у 55,0±4,5% пациентов (p<0,001). Итак, подключение модификатора лейкотриенов в комплекс базисной терапии детей с бронхиальной астмой среднетяжелого течения обеспечивает аддитивный клинический эффект, что позволяет использовать его как ”гормонсберегающее” средство, а также в качестве “терапии усиления”. Быстрая клиническая стабилизация на фоне антилейкотриеновой терапии может служить аргументом для назначения этой группы препаратов в период обострения астмы у детей. Также у детей, принимавших препарат Зеспира, было зарегистрировано достоверное клини- ческое улучшение аллергического ринита (RR=7,0;p=0,005).

The double blinded placebo-controlled clinical trial was aimed to evaluate the role of leukotriene modifiers in the management of pediatric asthma and to elaborate the optimal treatment conduit. Objectives of the study included the assessment of the additive effect and the decrease in steroids necessity in children with moderate persistent asthma receiving Montelukast (Zespira®), and the evaluation of the clinical efficiency for the atopic syndrome evolution. The study revealed that 12 weeks course of Zespira shows a threefold decrease of asthma exacerbations (p<0,01) and there was a statistically significant reduction in β2-agonists (p<0,01), which allowed a double reduction of the inhaled corticosteroides dose without interfering with the general condition in 55,0±4,5% patients (p<0,001). Thus, the combination of the leukotriene modifiers with the complex basic therapy in children with moderate persistent asthma assures an additive clinical effect that allows using them as “steroid-preserving” treatment, and also in quality of “strengthening therapy”. Rapid clinical stabilization seen at children receiving leukotriene modifi ers is a strong reason for the indication of this group of drugs in asthma exacerbation at children. Also, children receiving Zespira showed statistically signifi cant clinical improvement of allergic rhinitis (RR=7,0; p=0,005).
