Background. Chronic pancreatitis in children is a rare disease with reoccurring, fibrosing, progressive course, and frequent development of exocrine and endocrine pancreatic insufficiency. Objective of the study. To present a clinical case of an 11-year-old patient diagnosed with chronic pancreatitis in aggravation, mild form. Material and methods. Anamnestic, clinical and paraclinical data were taken from the inpatient medical record. The results obtained were compared with similar cases in the medical literature. Results. Eleven-year-old girl admitted with complaints of abdominal pain localized in the epigastric region, moderate intensity (Score 4p), repeated nausea and vomiting, abdomen painful on palpation in the epigastrium, absence of skin jaundice and hepatosplenomegaly, Mayo-Robson point positive. Investigations: complete blood count: Hb - 15.4 g/ dL; erythrocytosis (5.37x1012/l); lymphopenia; biochemical examination: minimal cytolysis (ALT - 44.6 U/l; AST - 77.8 U/l), total bilirubin - 14.54 mmol/l; moderate hyperglycemia (Glucose - 7.88 mmol/l); serum lipase - 32.0 U/l; serum amylase - 43.3 U/l; decreased serum Fe (6.7 mmol/l); general urinalysis: ketone bodies +++, increased urinary alpha amylase (421.64 U/l); pancreatic elastase 1 in faeces100 mcg/g; abdominal ultrasonography: increased echogenicity of pancreas, size according to age: 13x13x16mm. Medication treatment was performed. The patient was discharged with improvement. Conclusions. In children chronic pancreatitis is difficult to diagnose because it can evolve without episode of acute pancreatitis. The following criteria are sufficient to establish the definitive diagnosis: recurrent pain and hyperechogenicity of the pancreatic parenchyma on ultrasonography |
