2Institute of Mother and Child. Background. Idiopathic juvenile arthritis (JIA) is the most common rheumatic disease in children, and uveitis is the most common extra-articular manifestation. Uveitis is potentially life-threatening and thus poses a considerable risk of morbidity associated with decreased quality of life. Objective of the study. Correlation of data from the literature with the preliminary results of the prospective study for the analysis of eye damage in children with idiopathic juvenile arthritis. Material and Methods. The literature review included analysis of articles from the PubMed, Medline databases from 2017-2022 to the keywords: idiopathic juvenile arthritis, eye damage, and uveitis. The prospective study included 30 children diagnosed with JIA, with ocular impairment, aged < 16 years, hospitalized in the rheumatology department of the Mother and Child Institute. Results. The demographic analysis of the study subjects revealed: the mean age of onset of JIA was 6.95±4.43 years; and the onset of uveitis was 6.98±3.5 years. The mean duration between onset of JIA and onset of uveitis was 22.07±26.6 months. Female patients (80%) developed JIA associated with uveitis more frequently. The oligoarthritis (56.6%) appeared the most common one followed by FR-negative polyarthritis (23.3%). Serologically, the titer of ANA was positive in 26.6% of cases. The most common form of uveitis seen in combination with AJI was anterior uveitis (80%). At least 63.3% of patients reported an eye complication. Irreversible vision loss in 16.7% of cases. Conclusion. Uveitis has an insidious onset, chronic evolution and a high incidence of disabling complications. Due to the progression of the joint syndrome and the deterioration of the ocular apparatus, JIA can quickly lead a patient to a disability, including decreased visual acuity, often to complete blindness. |
