Background. Cerebellar pilocytic astrocytoma is the most common type of benign cerebellar tumor in children, with a 90% survival rate at 10 years, postoperative recovery treatment being difficult due to the cerebellar cognitive-affective syndrome (CCAS) and suprasegmental vegetative dysfunctions. Objective of the study. To study the frequency of CCAS and suprasegmental vegetative dysfunctions in patients with cerebellar tumors in the late postoperative period. Material and methods. CCAS and vegetative suprasegmental dysfunction in the late postoperative period (≥2 years after intervention) were evaluated in 31 children (25 with pilocytic astrocytoma, 6 with medulloblastoma) using Scales: GAD-7, PHQ-9, PDS-10, COMPASS 31, Cerebellar Cognitive Affective Syndrome Assessment Scale. Results. Severity of ataxia expression: 19% severely expressed, 39% - moderate, 42% - mild; orthostatic intolerance: pronounced - 26%, moderate - 55%, mild - 19%; vasomotor disorders - 94%; secretomotor disorders - 87%; gastrointestinal disorders: pronounced - 68%, moderate - 32%; urinary disorders 39%; pupil-motor disorders: pronounced - 55%, moderate 45%; expressive language deficit - 42%; difficulties in spontaneous conversation - 39%; attention deficiency or mental flexibility - 94%; emotional lability - 74%; exaggerated/ diminished behavioral reaction - 77%; thoughts without logic or paranoia - 26%; lack of empathy - 45%; aggression 33.3%. Conclusion. Neurovegetative symptoms in patients with cerebellar tumors in the distant postoperative period are present in 94% of cases. CCAS- Definite was diagnosed in 68%. Postoperative chemotherapy and radiation have a negative effect on brain function in children. |
