Minimal access surgical treatment of the giant biloma
at a polytrauma patient (case presentation)
The clinic case is represented by a polytrauma patient after a car crash with a liver injury
of the IV degree. After the surgery intervention on liver (hepatorraphy and mesh packs) the
patient developed a giant subdiafragmal biloma confirmed by USG and CT. The biloma was
ECO-guided punctured, then drained in the VII intercostal space on the right side with a bilumen
drainage. During the procedure neither complications were seen. The level of the bile was 200-
250 ml/24 hours. Fistulography made in three days after the drainage showed a considerable
decrease of the subdiafragmal leak. The external bile fistula closed at the 14th day. The patient
was hospitalized for 58 days in good physical condition. |
