Rezumat

Tratamentul cancerelor la copii a cunoscut progrese majore. Succesele terapeutice în tratamentul limfoamelor maligne Non-Hodgkin (LNH) la copii au urmat identificării anumitor subgrupe diagnostice cu anumite strategii terapeutice distincte [1, 3]. Grupele terapeutice de limfoame sunt: limfoame limfoblastice cu celule precursoare B sau T, limfoame cu celule B mature și limfoame cu celule anaplazice [9]. Diagnosticul corect al limfoamelor maligne Non-Hodgkin este crucial pentru încadrarea în aceste grupe. Conform datelor din literatura de specialitate, protocoalele utilizate în tratamentul leucemiilor acute limfoblastice la copii s-au dovedit a fi foarte eficiente și pentru LNH limfoblastice cu rate de supraviețuire fără evenimente de 60-80% [9, 10]. Pentru copiii cu limfoame cu celule B, curele de chimioterapie high-dose, repetate la intervale scurte de timp, determină rate de supraviețuire de pînă la 90%. În limfoamele anaplazice se obțin rezultate terapeutice similare, însă se înregistrează o rată crescută de recidive. Cu toate acestea, o astfel de terapie este asociată cu o toxicitate importantă. Astfel, realizarea unui echilibru între intensitatea tratamentului și riscul individual de recidivă este un obiectiv major al cercetărilor contemporane. O varietate de noi opțiuni terapeutice sunt în curs de evaluare la copii.

Major advances have been made in the treatment of childhood cancers. The recognition that different Non-Hodgkin Lymphoma (NHL) subtypes require different treatment strategies was fundamental to developing successful therapy regimens [1, 3]. Currently established therapy groups are lymphoblastic lymphoma (LBL) of precursor B or T-cell type, mature B-cell neoplasms (B-NHL), and anaplastic large cell lymphoma (ALCL)[9]. Accurate diagnostic classification is crucial for allocating patients to appropriate treatment groups. Therapy protocols designed to treat children with acute lymphoblastic leukemia (ALL) have proven highly efficacious and are associated with event-free survival rates up to 60-80% [9, 10]. For children with B-NHL, a strategy of rapidly repeated high-dose intense chemiotherapy courses proved more efficacious, with event-free survival rates up to 90%. In patients with ALCL, comparable results are achieved with either strategy, although this group has the highest relapse rate. The price of these efficacious treatments is considerable toxicity. Thus, the refinement of the balance between treatment burden and individual patient risk for failure is a major task. A variety of new treatment options await evaluation.

В лечении злокачественных нон-ходжкинских лимфом был достигнут значительный прогресс. Положительные результаты в лечении связаны с выделением определенных диагностических субгрупп (лимфобластные лимфомы, B-клеточные лимфомы и анапластические лимфомы), для которых разработаны различные терапевтические стратегии [1, 3, 9]. Точное определение морфологической формы злокачественной нон-ходжкинской лимфомы является ключевым моментом при выборе терапевтической субгруппы. Согласно данным литературы, протоколы, используемые в лечении острых лимфобластных лейкемий у детей, оказались очень эффективными и при лечении лимфобластных лимфом с частотой выживаемости без рецидивов -60-80%[9,10]. Среди детей с B-клеточными лимфомами, которым проводятся повторные курсы интенсивной химиотерапии (проведенные через короткие интервалы времени), частота выживаемости достигает до 90%. При анапластических лимфомах при подобных схемах, лечения достигаются схожие результаты, однако у этих пациентов регистрируются частые рецидивы. К сожалению цена эффективности такого лечения – это высокая токсичность препаратов. Вот почему необходимо найти идеальное равновесие между интенсивностью проводимого лечения и индивидуальным риском возникновения рецидива. В настоящее время изучаются новые терапевтические возможности в лечении данной патологии у детей.
