Background. The debate of orthodontic treatment, with or without extractions, was discussed more than a century. With usage of modern technics, the rate of extractions diminished. For streamlining of the proper treatment, the need of argumentation of extractional orthodontic treatment is primordial. Objective of the study. To determine the factors that influence the treatment plan, with or without extractions, in order to establish the stomatognathic system´s morphofunctional equilibrium. Material and Methods. A clinical study, based on 14 patients files was done. Selected patients had between 7 to 35 years and reported dento-maxillary anomalies. The study relied on assessment of photostatic analysis, pre-treatment study cast, lateral cephalometry and OPT records. Results. In the decision-making process, following parameters were evaluated: patient complaint, soft tissue profile, dental crowding, overbite, midline deviation, growth potential and pattern, incisor axes, curve of Spee, 3rd molar eruption space, vertical dimension. It was established that for the patients aged ≤12±2 years, growth potential was the most significant factor, and for patients aged ≥14±2 years the determinant parameters were: 85,71% patient profile, 78,57% dental crowding, 71,42% incisor axes. Conclusion. (1) Esthetic aspect, soft tissue profile and degree of dental crowding were the keys for decision-making regarding orthodontic treatment, with or without extractions. (2) Based on age and modern orthodontic treatment methods dental extractions are approached accordingly |
