Background. Status epilepticus (SE) is the most common neurological emergency in children, being a condition that results from the loss of mechanisms responsible for ending seizure or by initiating mechanisms that cause an abnormal seizure response. Objective of the study. The present study represents an analysis of the course of SE among children with "de novo" seizures and previously established epilepsy, by analyzing the type of seizures, EEG route and assessment of serum concentration of antiepileptic drugs. Material and Method. A retrospective study was performed by a single center (Hospital of Mother and Child Health Care) during the period between 2015-2019. The study included 115 children with convulsive SE, aged between 1 month and 18 years, hospitalized in the pediatric intensive care unit. Medical records were revised to obtain variables related to demographics and types of seizures. Results. Among the 115 children with SE, 72 (62.6%) had a previously established diagnosis of epilepsy. Focal seizures were present in 32.7% of cases, and 45.2% required intubation; the mortality rate being 0.9%. In children with a previously established diagnosis of epilepsy, a pathological EEG was more common (p <0.001). In 81% of children with pre-existing seizures, antiepileptic drug levels were known, but 58.6% of them had subtherapeutic levels. Conclusion. SE is more common in children with previously established epilepsy. SE with focal seizures often require intubation. Subtherapeutic serum concentrations of antiepileptic drugs have been noticed more frequently among children with SE in previously diagnosed epilepsy. |
