Background. Pelvic trauma is one of the most difficult problems in trauma surgery, and its prevalence is 3-7% of the total number of injuries of the musculoskeletal system. Spino-pelvic fixation is performed to restore the integrity of the vertically displaced posterior semi-ring. Objective of the study. Presentation and approval of the spino-pelvic stabilization method in polytraumatized patients with severe-unstable pelvic fractures and evaluation of surgical treatment results. Material and Methods. We analyzed 5 medical histories of patients with unstable type C pelvic fractures, who were operated and observed in dynamic. In 3 cases of communitive sacral bone fractures bilateral spinopelvic fixation were performed and in 2 cases unilateral fixation combined with ilio-sacral screw were performed. Results. After spino-pelvic stabilization, three patients regained their mobility and were transferred to ambulatory treatment 14 days after hospitalization. 2 patients with signs of contusion of the sacral plexus in which the decompression of the nervous structures were performed, were hospitalized for 31 and 42 days. A patient with signs of inflammation of the postoperative wound needed to change the osteosynthesis material. Satisfactory bone block was formed in all patients. All patients returned to work. Conclusion. 1) Spino-pelvic fixation is an effective method of internal osteosynthesis in pelvic fractures with vertical displacement and can be used successfully in both trauma and other pathologies. 2) Spino-pelvic fixation allows a firm stabilization and early mobilization of the patient. |
