Background. Cardiac injuries (CIs) are an important case of mortality, survival rates ranging from 19% to 74% in specialized institutions. Objective of the study. Evaluation of treatment results in patients with heart wounds. Material and Methods. Retrospective study, 2005-2020, EMI, 41 trauma patients, M:W/19,5:1, mean age 45,8±8,9years. Gunshot wounds –7(17,1%) cases, stab wounds –34(82,9%). Investigations performed: EKG – 22(53,7%), chest x-Ray –22(53,7%), FAST –2(2,9%), pleurotomy – 18(43,9%), pericardiocentesis –2(4,9%), thoracoscopy –1(4,6%). Results. At admission, 36(87,8%) patients were hemodynamically unstable, among them 19(52,8%) were immediately transferred to the OR. Mean interval between hospitalization and intervention –26,6±6,3min. Surgical access: left anterolateral thoracotomy(ALT) –26(63,4%), right ALT –13(31,7%), left posterolateral thoracotomy – 2(4,9%). Operative findings: nonpenetrating injuries –5(12,2%): pericardium-3, myocardium-2; penetrating injuries –36(87,8%): RV-16, LV-11, RA-6, LA-1, RAA-1, and SVC-1. Associated thoracic injuries –29(70,7%): lung parenchyma –25(86,2%), internal mammary artery –3(10,3%), intercostal artery –1(3,5%). Length of stay –13,2±4days, including ICU –2,9±1,2. Lethality – 7(17,1%). Conclusion. Cardiac suture performed in time and severity of cardiac injury determined the survival rate of 82,9%. Lethality increases directly proportional to the gravity of heart lesion, volume of blood loss and lesion in right vs left heart compartments (p<0,05). |
