Background. Obesity is in a spectaculous increase over the last decades. Bariatric surgery is the only option to significant and sustained weight loss and to improvement of comorbidities and mortality decrease. Efficiency of bariatric surgery is well-studied, but its complications have been underestimated. Objective of the study. Elaboration of both diagnostic and treatment algorithm for early postoperative complications after Roux-en-Y gastric bypass. Material and Methods. The clinical trial is based on the evaluation of early postoperative results of 433 patients who supported Roux-en-Y gastric bypass during 2009-2020. Weight varied from 80 to 245 (121,8 ± 26,3) kg, age was of 18-68 (40,5±10,4) years, sex ratio was 3,5/1, with the prevalence of women. Results. Early postoperative complications were recorded in 44 (10,1%) cases. The complications mostly consisted of bleeding of the transit excluded stomach – 17 (38,6%) cases, wound suppuration – 11 (25%) cases and acute gastric dilatation – 6 (13,6%) cases. All patients (n=6) with acute gastric dilatations were reoperated via parietal gastrostomy. Conclusion. Postoperative complications after Roux-en-Y gastric bypass are dominated by bleeding of the transit excluded stomach. Gastric acute dilatations is a medical emergency, the solution being parietal gastrostomy. |
