Background. Amiodarone induced pulmonary toxicity vary between 4-17% and is determinated by the cumulative dose, age, pre-existing lung disease, general anesthesia, oxygen administration, surgical procedures manifested by alveolar / interstitial pneumonitis, interstitial fibrosis, respiratory distress. Objective of the study. Analysis and systematization of the pathogenetic mechanisms of amiodarone-induced pulmonary toxicity. Material and Methods. Articles from the PubMed database were selected and analyzed according to the keywords „ amiodarone”,”pulmonary toxicity”, “idiopathic fibrosis”. Results. Clinical symptoms are bronchospasm, exacerbation of asthma, interstitial pneumonia, eosinophilic or lupoid, diffuse alveolar hemorrhage, pulmonary nodules, acute respiratory distress. The pathogenesis of the lung lesion was estimated to be determined by the direct toxic effect on intracellular phospholipids, inflammatory and immune processes (polymorphonuclear infiltration of lymphocytes and foamy macrophages, cytoplasm vacuolation, type II pneumocyte hyperplasia, interstitial thickening). It has been shown that pulmonary toxicity can be caused by the activation of lipid peroxidation with damage to the cell membrane of vessels, decreased surfactant synthesis and increased fibrosis. Conclusion. Amiodarone and desethylamiodarone have been shown to be responsible for pulmonary toxicity due to high lipophilicity, storage in lung tissue, high half-life and cumulative dose. |
