Introduction. Perinatal HBV infection has a risk of chronicity of up to 90-95% in the first year of life of the child. Objective of the study. Presentation of a difficult diagnosis case with a poor prognosis. Materials and methods. The data were obtained as a result of the analysis of medical records of the child, medical records of patient’s discharge, post-mortem morpho-histological research of liver tissue, including: epidemiological data of the child’s family and mother’s concerning the risk of HBV infection, morbid status of the mother in pre- and during current pregnancy, history of the disease in the fi rst days aft er the birth of the newborn, up to the current hospitalization; clinical-paraclinical data: blood biochemistry, markers of HBV and HCV, abdominal sonography, CT with contrast of the bile ducts. Results. Infection of the child with HBV probably occurred intrauterine or intranatally with the onset of symptoms such as: prolonged neonatal jaundice and acholic stools, then cytolytic syndrome and detection of HBs Ag at 3 months of age. Conclusions. HBV infection of the 5-month-old child occurred in the perinatal period, and the bile duct atresia during embryogenesis, the source of HBV infection served the mother of the child, who suff ered from childhood with HVBC, HBsAg positive form. |
