Цель исследования: изучить динамику выраженности депрессии и тревоги в сочетании с биоэлектрической активностью мимических мышц у больных с первичным гипотиреозом под влиянием лечения L-тироксином. Материал и методы. Для исследования выбирали больных с первичным гипотиреозом (ПГ), женского пола, в возрасте от 20 до 60 лет, без выраженных сердечно-сосудистых заболеваний и другой выраженной сопутствующей патологии. Отбирали больных без патологических изменений классической ЭКГ. Для исследования больные были разделены на 2 группы: 1) больные с ПГ с легкими или транзиторными проявлениями субклинической ишемии миокарда (индекс миокарда, IM=16- 23%), лечение L-тироксином проводили в дозе 1,6 мкг/кг массы тела (n=16); 2) больные с ПГ с умеренными субклиническими проявлениями ишемии миокарда (IM 24-42%), в этой группе L-тироксин назначали в дозе 0,6 мкг/кг массы тела. Для определения выраженности тревоги и депрессии использовали Hospital Anxiety and Depression Scale (HADS). Биоэлектрическую активность m.frontalis и m.orbicularis oris определяли с помощью диагностического комплекса EMG – Мicro (Neurosoft, Россия). Степень ишемии миокарда определяли с использованием Cardio-Visor-06S – ЭКГ- dispersing mapping. Результаты. В 1-й группе пациентов под влиянием лечения (на 60-й день) отмечали выраженный противотревожный эффект и слабо выраженное антидепрессивное действие, в сочетании с сохранением дискоординации биоэлектрической активности m.frontalis и m.orbicularis oris (фронто-оральный индекс существенно уменьшался: до лечения – 0,809±0,008, на 60-й день лечения – 0,701±0,009, р˂0,001). IM имел тенденцию к нарастанию (до лечения – 9,8±1,16%, на 60-й день лечения – 12,9±1,01%, р˂0,05). Во 2-й группе на фоне лечения отмечали выраженное противотревожное и антидепрессивное действие, в сочетании с сохранением дискоординации биоэлектрической активности мышц лица (фронто-оральный индекс существенно возрастал: до лечения – 0,621±0,007, на 60-й день лечения – 0,728±0,005, р˂0,001). IM существенно не изменялся, имея тенденцию к увеличению. Выводы. 1. Различные дозы L-тироксина оказывают дифференцированное влияние на психо- эмоциональное состояние и биоэлектрическую активность мимических мышц лица. 2. У больных с ПГ выбор терапевтической дозы L-тироксина зависит от выраженности ишемических расстройств в миокарде. 3. Для мониторинга больных с ПГ в процессе лечения рекомендуется использовать ЭКГ- dispersing mapping с целью своевременного выявления субклинических ишемических нарушений в миокарде.
Aim of the research. To study the dynamics of depression and anxiety in association with the bioelectric activity of mimic muscles in patients with primary hypothyroidism (PH) under the influence of L-thyroxine. Material and Methods. Female patients with PH, aged 20 to 60 years, without severe heart diseases or other comorbidities were selected for research . Patients without pathological signs on ECG were selected. They have been divided into 2 groups, in order to carry out the tests: 1) hypothyroid patients with mild or transitory manifestations of subclinical myocardial ischemia (myocardial index, MI = 16-23%). A dose of 1.6 mcg/kg body weight of L-thyroxine was indicated (n = 16) hypothyroid patients with moderate subclinical manifestations of myocardial ischemia (МI = 24-42%). L-thyroxine was indicated at a dose of 1.0 mcg/kg body weight. The severity of anxiety and depression was determined using Hospital Anxiety and Depression Scale (HADS). Bioelectric activity of m.frontalis and m.orbicularis oris was determined by EMG–Micro diagnostic system (Neurosoft, Russia). The degree of myocardial ischemia was determined using Cardio-Visor-06S – ECG- dispersing mapping. Results. In the 1st group of patients an important anti-anxiety effect and poorly marked antidepressant effect were observed under treatment (60 days), in association with preservation of bioelectrical activity discoordination of m.frontalis and m.orbicularis oris (the fronto-oral index was significantly decreasing: before treatment – 0,809 ± 0,008, on the 60th day of treatment – 0,701 ± 0,009, p˂0,001). MI tended to increase (before treatment – 9,8±1,16%, on the 60th day of treatment - 12,9±1,01%, p<0,05). In the 2-nd group, an important anxiolytic and antidepressant effects were observed under treatment, combined with the preservation of bioelectric activity discoordination of facial muscles (the fronto-oral index was significantly increasing: before treatment – 0,621 ± 0,007, on the 60th day of treatment – 0,728 ± 0,005, p˂0,001). MI did not significantly change, but it tended to increase. Conclusions. 1) Various doses of L-thyroxine have a differential impact on the psycho-emotional state and bioelectric activity of facial muscles. 2) In patients with PH, the therapeutic dose of L-thyroxine is dependent on the severity of myocardial ischemic disorders. 3) ECG- dispersing mapping is recommended for treatment monitoring of patients with PH for timely detection of subclinical ischemic disorders of the myocardium.
