Rezumat

Introducere: Tratamentul hemoragiilor de etiologie portală reprezintă una din cele mai actuale probleme a chirurgiei contemporane. Se observă o tendință de creștere considerabilă a incidenței cirozei hepatice – cea mai frecventă cauză a hipertensiunii portale. Drept consecință crește și frecvența complicațiilor, îndeosebi a hemoragiilor din varicele esofagiene și gastrice. Material și metode: Au fost incluși 132 de pacienți (B-79,F-53) cu hemoragie din varice esofagiene (VE) care au beneiciat de ligaturare endoscopică(LE). Etiologia sindromului de hipertensiune portală a fost: ciroză hepatică postvirală (CH)(n=118), tromboza sistemului portal (TSP)(n=3), CH+carcinom hepatocelular(n=2), CH+TSP(n = 9). Pacienții au fost repartizați conform rezervei funcționale hepatice (Child - Pugh): А(12), В(56), С(64). Rezultate: Hemoragie activă la momentul ligaturării a fost în 58(43,9%) cazuri, inclusiv hemoragie în jet – 27(20,4%) și prelingere – 31(23,5%). Stigmate endoscopice ale hemoragiei stopate - 74(56 %) pacienți: cheag ixat pe VE – 21(15,9%) și ibrină – 53(40,1%) cazuri. Pentru LE au fost utilizate seturi GF – OVL 510(Belgia) în 127 cazuri și MBL – 10 (Wilson-Cook, USA) – în 5. În caz de vizualizare a varicelui hemoragic, acesta era ligaturat (n=78,59%) cu aplicarea ulterioară a inelelor pe celelalte varice. În 54(40,9%) cazuri, când sursa hemoragiei nu putea i depistată a fost aplicată metoda de LE „spiralat – în tablă de șah”. Sesiunile elective de LE au fost efectuate peste 4 săptămâni până la obliterarea completă a VE sau până la micșorarea VE până la dimensiunile ce nu permit aspirarea varicelor în camera de ligaturare (F0-1, RCS (-), conform Japan Research Society of Portal Hypertension, 1991). Eiciența LE în stoparea hemoragiei din VE a constituit 97,8%. În total în acest lot de pacienți au fost efectuate 237 de sesiuni de LE cu cifra medie de 1,71±0,1 (de la 1 la 6) per pacient. În medie au fost aplicate 9,3±0,5(1-36) inele per pacient. Concluzii: 1) LE este o metodă eicientă de stopare a hemoragiilor active din VE și pentru proilaxia recidivelor. 2) LE are un număr redus de complicații. 3) La utilizarea acestei metode se constată o rată înaltă de obliterare a VE cu un număr relativ redus de sesiuni de LE.

Background: Treatment of the bleeding of portal etiology is one of the most actual problems of modern surgery. There is an increasing incidence of liver cirrhosis – the most common cause of portal hypertension. Consequently, complications’ incidence increases, especially bleeding from esophageal and gastric varices. Methods and materials: A total of 132 patients (M-79, F-53) with esophageal variceal(EV) bleeding, who underwent endoscopic band ligation(EBL) were included. Etiology of the portal hypertension: viral liver cirrhosis (LC)(n=118), portal system thrombosis (PST)(n=3), LC+ hepatocellular carcinoma(n=2), LC+PST(n=9). According to liver function (Child-Pugh) patients were distributed: А (12), В(56), С(64). Results: Active bleeding during EBL in 58(43.9%) cases, including spurting bleeding – 27(20.4%) and oozing – 31(23.5%). Endoscopic stigmata of recent bleeding - 74(56 %) patients: adherent thrombus on the EV – 21(15.9%) and ibrin – 53(40.1%) cases. We used for EBL: GF–OVL 510(Belgium) in 127 cases and MBL–10(Wilson-Cook, USA) – in 5. In case if the bleeding varix was identiied it was ligated (n=78.59%) with subsequent ligation of the rest of the varices. If the bleeding source could not be identiied in 54(40.9%) cases, EBL was performed in a “spiral-chess” pattern. Elective EBL was performed after 4 weeks until complete EV obliteration or until EV shrank to the size which does not allow its suction into the ligation chamber (F0-1, RCS (-) according to Japan Research Society of Portal Hypertension, 1991). EBL was eicient for hemostasis in 97.8%. In this group of patients a total of 237 EBL sessions were performed with a median of 1.71±0.1 (1-6) per patient. The median number of bands applied was 9.3±0.5(1-36) per patient. Conclusion: 1) EBL is an eicient method of hemostasis in EV bleeding and for recurrence prophylaxis 2) EBL carries a low risk of complications. 3) There is a high rate of EV obliteration after this method with a reduced number of EBL procedures.
