Background. Nowadays, 24-hour ambulatory blood pressure monitoring (ABPM) is widely used, which provides a fuller picture of blood pressure (BP) values and its variability during the day and night. Materials and methods: Eighty children aged 10-18 years with high BP values were enrolled in the study. In comparison, 31 children with normal BP values were interpreted. The ABPM was performed by DIASYSOFT version 4.6.4. The following indices were analyzed: 1. the average values for BP during the day, night and 24 hours; 2. time index; 3. circadian variations of BP; 4. the type of diurnal BP profile. Results: Mean values of BP are significantly higher in hypertensive, versus normotensive children (133,01±0,68/74,3±0,77 versus 106±5,07/67,7±1,24 mmHg). Hypertensive children in 48 (60,1%) of cases had increased pulse pressure but normotensive patients had only optimal values. The patients from the baseline group in 45,6% of cases had high variability of BP in 24 hours; in 38% of cases – daytime BP; in 3,9% of cases – night-time BP. In the patients of the control group was not detected any variability above the normal values of the BP. From all the patients with increased morning BP, in 83,3% of cases, its increasing speed was above the normal limits, in comparison to 9,2% of cases in the children who had normal-increased morning BP (p<0,01). In hypertensive patients, the non-dipper day profile was found in 51 (63,7%) patients versus 5 (16,1%) (p<0,01); The diurnal dipper-type profile (normal) was found more frequently in normotensive children (n=26; 83,9%) versus hypertensive children (n=24; 30%, p<0,01). The night-picker daytime profile was reported only in the baseline group, and the over-dipper profile was registered in 1 (3,2%) normotensive child. Conclusion: Hypertensive children more frequently have been found with high variability of BP and high pulsatile pressure, with an increased BP morning rate and with a non-dipper pathological profile. These values have an unfavorable prognosis in adulthood cardiovascular pathology. |
