Introduction: Data on the treatment of West syndrome (SW) are controversial. Purpose: clinical-electroencephalographic analysis of children with SW subjected to Prednisolone therapy, to assess its effectiveness. Material and methods. In aa. 2019-2022, at the Pediatric Neurology Clinic of the USMF Pediatrics Department “Nicolae Testemitanu” 16 children with SW were evaluated. All patients received treatment with Prednisolone at doses of 5 to 8 mg / kg in 24 hours for 7 to 14 days, further decreasing the dose of the drug. The effect of the treatment was appreciated by ameliorating the epileptic seizures and the EEG pathways. Statistical processing with the Quanto program. Results. Of the 16 children with SW, 5 (31.25%) had consequences of perinatal hypoxic-ischemic lesions, 3 (18.75%) — consequences of CNS infections, 3 (18.75%) — brain development abnormalities, 2 (12.5%) — innate errors of metabolism, in 3 (18.75%) cases the cause was unknown (molecular-genetic examinations followed). EEG recordings were performed at the beginning of treatment, then at 7 and 14 days of Prednisolone administration. EEG at onset: hypsarhythmia typical variant — 9 (56.3%) and modified — 7 (43.7%). At 7 days of therapy: clinical-electrographic improvement in 15 (94%) children, in one — non-essential improvement (the presence of major CNS abnormalities). At 14 days: clinical appearance and EEG with a positive result in all cases. Complications: 2 (12.5%) children — psychomotor agitation and abdominal pain. Conclusions. Corticosteroid therapy is effective in the treatment of epileptic spasms and SW. Recognition of the disease at an early stage allows an early positive diagnosis and an appropriate therapeutic attitude. |
