Rezumat

Introducere: Cancerul mamar (CM) este o maladie cu incidenţă în creştere. Unele forme de CM prezintă dificultate în diagnostic, fiind necesar pe lângă examenul clinic şi un examen imagistic complex ca mamografia (MG), ultrasonografia (USG) şi rezonanţa magnetică (IRM). Material: 156 paciente (100%) examinate mamografic cu scop de screening au fost suspectate pentru CM (BI-RADS-4). Luând în consideraţie unele incertitudeni în diagnosticul acestui lot de paciente, au fost investigate suplimentar. Examenul USG a confirmat BI-RADS-4 în 91 cazuri (58.4%) la 6 (3.8%) paciente - BIRADS5, la 44 (28.2%) paciente nu s-a confirmat (BI-RADS-3) şi recomandat control repetat peste 2-3 luni. În 15 (9,6%) cazuri, cu tablou MG şi USG neclar, s-a efectuat IRM cu contrast. Prin investigaţia IRM din 15 paciente la 3 paciente s-a confirmat CM (BI-RADS-5), iar la 12 paciente s-a infirmat (BI-RADS-2). În rezultatul examenului imagistic complex a pacientelor suspectate pentru CM, au fost confirmate 100 (64.1%) cazuri de CM (BIRADS5), ulterior confirmate morfologic prin tratament chirurgical. Concluzii: Suspiciune de cancer mamar la examenul mamografic (BI-RADS-4) este indicaţie, de facto, pentru intervenţie chirurgicală. Examenul imagistic complex a glandelor mamare prezintă o importanţă sporită, acesta permiţând confirmarea sau infirmarea diagnozei de CM, delimitarea hotarelor şi gradului afectării loco-regionale cu ghidarea ulterioară a chirurgilor oncologi în tactica operatorie şi tratament.

Introduction: Breast cancer (BC) is a growing disease. Some forms of BC have difficulty in diagnosis, requiring a complex imaging examination such as mammography (MG), ultrasonography (USG) and magnetic resonance imaging (MRI) besides the clinical examination. Material: 156 patients (100%) examined mammographically for screening purposes were suspected for CM (BI-RADS-4). Taking into account some uncertainties in the diagnosis of this group of patients, they were further investigated. The USG exam confirmed BI-RADS-4 in 91 cases (58.4%) in 6 (3.8%) patients - BI-RADS5, 44 (28.2%) patients not confirmed (BI-RADS-3) repeated over 2-3 months. In 15 (9.6%) cases with unclear MG result and USG, contrast MRI was performed. The MRI investigation of 15 patients confirmed BC in 3 patients (BI-RADS-5), and 12 patients BC denied (BI-RADS-2). As a result of the complex imaging examination of BC suspected patients, 100 (64.1%) BC cases (BI-RADS-5) were confirmed, subsequently confirmed morphologically by surgical treatment Conclusions: Breast cancer suspected in mammographic screening (BI-RADS-4) is, de facto, an indication for surgery. The complex imaging examination of the mammary glands is of great importance, allowing the confirmation or refutation of the BC diagnosis, the delimitation of the affected area borders and degree of loco-regional involvement with subsequent guidance for oncologist surgeon in surgical tactics and treatment.
