Background. Osteoporotic fractures are common in patients with chronic kidney disease. Morbidity and mortality are higher in chronic kidney disease patients with a fracture compared to general population. Objective of the study. Synthesis of current studies on the role of vitamin D in treatment of osteoporosis in patients with chronic kidney disease. Material and Methods. A review of the literature was conducted in which the search source was the MEDLINE database. Information was obtained using the PubMed, HINARI engine along with the PsychArticles database. Results. Vitamin D is the inactive precursor of 1,25dihidroxyvitamin D and exists in two forms, vitamin D3 (cholecalciferol) and D2 (ergocalciferol). Vitamin D deficiency is common in patients with chronic kidney disease, particularly in patients with proteinuria, due to loss of 25-hydroxyvitamin D and its binding protein, and represent an important factor in the development of secondary hyperthyroidism. The advantage of native vitamin D is maintenance of feedback mechanisms and possible effects due to extrarenal hydroxylation. Conclusion. Cholecalciferol is an excellent therapeutic alternative to treat patients with chornic kidney disease, as a result it leads to a significant increase in plasma 25 hidroxyvitamin D concentrations and reduction of PTH. |
