Background. Spontaneous pneumothorax is a complex clinical syndrome and occurs in children aged 15-18 years. Clinical symptoms include 2 cardinal signs such as chest pain and dyspnea. Pneumothorax may be associated with spontaneous hemothorax and bleeding and hypovolemic shock. Objective of the study. The therapeutic attitude has two important objectives to ensure pulmonary re-expansion with the avoidance of recurrences by pleural drainage or by classical surgery. Material and Methods. Totally diagnosed type III (lung bubbles with dimensions of 1.5 - 2.0 cm), and type IV (bubbles over 2 cm spread over the entire lung surface). In 3 cases pneumothorax was associated with pulmonary hemorrhage. Surgical treatment included pleural drainage, electrocoagulation of small bubbles, and thoracotomy with surgical resolution. Results. Conclusion. The algorithm developed and proposed for diagnosis and surgical treatment allowed the improvement of the lung anatomical structure. Total pleurotomy with bubble resection and electrocoagulation remain standard methods of treatment in spontaneous pneumothorax to prevent recurrences. |
