Background. Endometriosis of the postoperative scar (EPS) of the anterior abdominal wall (AAW) after obstetric interventions (OI) is an orphan pathology characterized by the presence of endometrial and stromal cells at this level. The AAW is the most common location extragenital endometriosis. Objective of the study. The evaluation of the particularities of EPS of AAW after OI depending on the approach and the type of intervention. Material and Methods. Retrospective – prospective study on a group of 31 patients with EPS. Results. 28 patients(90.4%) with EPS after csection: Pfannenstiel laparotomy–26, median –2. Average age: 30.8±1.1 years(95% CI:28.63–32.87). EPS after planic surgery – 71.4%(n=20), in 28.5% (n=8) – in emergency. The average term from the CS to the intervention for EPS – 46.1±4.1 months (95% CI:37.78–54.37). Involvement of PAA layers: subcutaneus – 7(25%), subcutaneus+aponeurosis – 12(42.8%), subcutaneus+aponeurosis+muscles – 5(17.8%), muscles – 5(17.8%). Resections R0 – in all cases. The tension – free method in 60.7%(n=17).The diagnosis was confirmed by histological and immunohistochemical examination. Average period of postoperative monitoring – 27.7±2.1 months without any recurrences Conclusion. EPS is a rare pathology, difficult to diagnose preoperatively, with some pathognomonic symptoms. There is a higher frequency of EPS in case of planic interventions. The prognosis is favorable and the possibilities of resection R0 ensure a considerable decrease in the frequency of recurrences. |
