Medullary spongy kidney (MSM), also called Lenarduzzi-Cacchi-Ricci disease, is a rare congenital anomaly characterized by cystic dilation of the medullary collecting ducts, manifested by nephrocalcinosis, urolithiasis, distal renal tubular acidosis, recurrent urinary tract infections and reduction of bone mineralization.Complications such as nephrocalcinosis and recurrent urinary tract infections associated with MSM can sometimes lead to the development of chronic kidney disease. In this article, the authors present the clinical case of a 7-year-old boy, admitted to the Nephrology Clinic of the Mother and Child Institute with complaints of dull pain in the bilateral lumbar region and recurrent urinary tract infections. As a result of imaging investigations by intravenous urography and computed tomography of the kidneys in angiographic regime, signs of bilateral medullary nephrocalcinosis were detected, characteristic for medullary spongy kidney and bilateral renal artery. Th erapy with potassium citrate and thiazide diuretics has been shown to be eff ective in stabilizing nephrocalcinosis and preventing the formation of kidney stones, having an important role in regulating calcium homeostasis and bone mineralization. |
