Rezumat

Introducere. Tratamentul hemopatiilor maligne include: chimioterapie, radioterapie, imunoterapie, care induc o imunosupresie şi, respectiv, prezintă un risc major de dezvoltare al infecţiilor. Factori potenţiali de dezvoltare a infecţiilor grave sunt: vârsta înaintată, imunodeficienţa secundară, leucopenia, neutropenia. Infecţia COVID-19 are o evoluţie mult mai gravă la pacienţii imunocompromişi cu hemopatii maligne. Forma gravă se dezvoltă de 2-3 ori mai frecvent, decât la persoanele sănătoase. Mortalitatea pacienţilor cu hemopatii maligne în asocierea infecţiei SARS-CoV-2 este mai mare de 20%. Pacienţii cu hemopatii maligne au un potenţial înalt pentru vindecare la efectuarea unui tratament antitumoral intensiv, cu evitarea ulterioară a complicaţiilor infecţioase, spre deosebire de pacienţii fără o terapie în deplin volum. Totodată tratamentul hemopatiilor maligne este necesar de efectuat la timp şi în volum deplin. Întârzierea iniţierii chimioterapiei poate afecta negativ prognosticul pacienţilor cu hemopatii maligne. Măsuri profilactice specifice pentru infecţia SARS-CoV-2 la pacienţii cu hemopatii maligne nu sunt. Este necesar de a efectua măsurile generale, care includ măsurile sanitaro–igienice, izolarea la domiciliu, la posibilitate activarea de la domiciliu şi testarea pentru infecţia SARS-CoV-2. Obiectivul acestui articol este prezentarea managementului hemopatiilor maligne în condiţiile pandemiei COVID-19. Material şi metode. Pentru a selecta date din literatură, a fost folosită baza de date PubMed, utilizând cuvintele cheie: ,,COVID-19”, ,,hemopatii maligne”, ,,pandemie”, ,,management”. Astfel, a fost sintetizată informaţia despre managementul hemopatiilor maligne în condiţiile pandemiei COVID-19. Rezultate. Tratamentul hemopatiilor maligne este necesar de efectuat la timp şi în deplin volum, deoarece întârzierea iniţierii terapiei poate afecta negativ prognosticul pacienţilor cu hemopatii maligne. Tratamentul specific antitumoral (chimioterapia, imunoterapia, radioterapia) este însoţit de citopenii şi toxicităţi semnificative, care trebuie să fie considerate ca factori de risc în infecţia SARS-CoV-2 şi, ca urmare, cu un efect negativ al evoluţiei bolii. Astfel, administrarea chimioterapiei poate agrava hipersensibilitatea pneumoniei indusă de COVID-19. În perioada pandemiei este necesar de a selecta pentru utilizare acele scheme de tratament, care ar limita vizitele pacienţilor la medic şi acordarea priorităţii administrării preparatelor perorale sau subcutane. Concluzii. Managementul corect al hemopatiilor maligne în condiţiile pandemiei COVID-19 va reduce numărul cazurilor de infectare şi evoluţia procesului infecţios. Este obligatorie gestionarea justă al posibilelor complicaţii infecţioase, care se pot dezvolta în perioda sau după administrarea tratamentului specific.

Introduction. The treatment of hematological malignancies includes chemotherapy, radiotherapy, immunotherapy that induce immunosuppression and, respectively, present a major risk of developing infections. Potential factors for the development of severe infections are: advanced age, secondary immunodeficiency, leukopenia with neutropenia. SARS-CoV-2 infection has a much more serious course in immunocompromised patients with malignant hematological diseases. The severe form develops 2-3 times more frequently than in healthy people. Mortality in patients with hematological malignancies associated with SARS-CoV-2 infection is greater than 20%. Patients with malignant hematological diseases have a high potential for healing when performing intensive antitumor treatment with subsequent avoidance of infectious complications as opposed to patients without full-volume therapy. At the same time, the treatment of malignant hematological diseases needs to be performed on time and in full volume. Delayed initiation of chemotherapy may adversely affect the prognosis of patients with malignant hematological diseases. There are no specific prophylactic measures for SARS-CoV-2 infection in patients with hematological malignancies. It is necessary to carry out general measures, which include sanitary-hygienic measures, isolation at home, possible activation at home and testing for SARS-CoV-2 infection. The objective of this article was to present the management of malignant hematological diseases in the conditions of the COVID-19 pandemic. Material and methods. The PubMed database was used in order to select the data from the literature, using the key words: “COVID-19”, “hematological malignancies”, “pandemic”, “management”. Thus, the information containing the data on the management of malignant hematological diseases in the conditions of the COVID-19 pandemic. Results. The treatment of hematological malignancies needs to be performed on time and in full amount, because the delay of initiating the therapy can negatively affect the prognosis of patients with malignant hematological diseases. Specific antitumor treatment (chemotherapy, immunotherapy, radiotherapy) is accompanied by cytopenias and significant toxicities, which must be considered as risk factors in SARS-CoV-2 infection and as a result of a negative effect on the course of the disease. Thus, the treatment with chemotherapy may aggravate the hypersensitivity induced by COVID-19 pneumonitis. During the pandemic it is necessary to select for use those treatment schemes, which would limit the visits of patients to the doctor and to give priority to the oral or subcutaneous drug administration. Conclusions. Proper management of hematological malignancies in pandemic with COVID-19 will reduce the number of cases of infection and the evolution of the infectious process. At the same time, it is mandatory to follow a correct management of possible infectious complications, which may develop during or after specific treatment.
