Introduction. To describe the epilepsy evolution, the anamnestic particularities, the beginning, the type and frequency of epileptic seizures and antiepileptic treatment there were studied pregnancy and delivery in women with epilepsy. Materials and methods: The transversal retrospective study included analysis of 153 deliveries in women with epilepsy that took place in Institute of Mother and Child Care, Chisinau, Republic of Moldova, during 2010-2014. Results: During the study it was found out that the beginning of the epilepsy prevails in puberty, in 88 cases (57,5 ± 4,0%). In 2 cases (1,3 ± 0,9%) it was observed primary appearance of the epilepsy in pregnancy, which shows us a gestational epilepsy. In the patients under study it was observed the predominance of the structural- metabolic (symptomatic) form of the disease – 103 cases (67,3 ± 3,8%). This study showed that in women with frequent seizures of epilepsy before the pregnancy – 43 (22,1 ± 3,4%), the frequency of seizures during pregnancy did not change – 41 (26,8 ± 3,6% ). During pregnancy 69 (45,1 ± 4,0%) patients did not take antiepileptic treatment, 15 ( 9,8% ± 2,4) canceled the treatment on their own, and in 23 (15 ± 2,9%) patients the antiepileptic therapy was primarily prescribed during pregnancy, due to primary appearance or increasing frequency of epileptic seizures. And only 61 (39,9 ± 4,0%) women strictly followed the epileptologist’s recommendations. In 3 (2,0 ± 1,3%) cases the birth process was complicated by the emergence of tonic-clonic seizures, and in 8 (5,2 ± 1,8%) cases epileptic seizures were noted in the early postpartum period. Conclusions: Analysis of own data indicates that increased frequency of the epileptic seizures was observed in pregnant women which did not take any antiepileptic treatment or which canceled the treatment on their own. The postpartum period is vulnerable in women with epilepsy due to disturbed nighttime sleep, the presence of dolor syndrome and psycho-emotional stress, as well as the changes in pharmacokinetics of antiepileptic drugs and low compliance of mother to the dosing regimen. |
