Acest lucru se datorează faptului că nu există încă un remediu pentru virus, iar mortalitatea în rândul bolnavilor crește. Scopul lucrării. А evalua posibilitatea și eficacitatea transplantului de celule stem hematopoietice cu mutația genei CCR5. Material și metode. Studiul publicațiilor științifice și familiarizarea cu lucrările organizațiilor de cercetare. Rezultate. Această procedură implică înlocuirea celulelor stem bolnave cu cele sănătoase care au o mutație delta32 în gena CCR5. Acest lucru duce la absența proteinei CCR5 pe suprafața celulei și împiedică pătrunderea HIV în celulă. În total, 3 operațiuni de succes au fost efectuate în lume. Un exemplu al unuia dintre ei: un pacient este un bărbat de 40 de ani diagnosticat cu leucemie mieloidă acută. A fost diagnosticat cu infecție cu HIV, bărbatul este supus unei terapii antiretrovirale foarte active. Pacientul a suferit un transplant de celule stem alogene ale celulelor sanguine periferice CD34 de la un donator HLA-identic, care a fost testat pentru homozigotism cu alela CCR5delta32. Transferul a fost finalizat cu succes. Din momentul transplantului, pacientul nu necesită utilizarea terapiei. Concluzii. În ciuda succesului operației, această metodă de tratament este încă dificil de implementat, deoarece transplantul se efectuează numai în prezența bolilor hematologice maligne, este necesară o histocompatibilitate ridicată a celulelor donatoare și primitoare.

Background. HIV infection is a disease caused by the human immunodeficiency virus. This is the most common and urgent problem of humankind. This is because there is no cure for the virus yet, and mortality among sick people is increasing. Objective of the study. To evaluate the possibility and effectiveness of hematopoietic stem cell transplantation with CCR5 gene mutation. Material and methods. Study of scientific publications and familiarization with the work of research organizations. Results. This procedure involves replacing diseased stem cells with healthy ones that have a delta-32 mutation in the CCR5 gene. This leads to the absence of CCR5 protein on the cell surface and prevents HIV from entering the cell. In total, 3 successful operations were carried out in the world. An example of one of them: a patient is a 40-year-old man diagnosed with acute myeloid leukemia. Previously diagnosed with HIV infection, the man is undergoing highly active antiretroviral therapy. The patient underwent a transplant of allogeneic stem cells of peripheral blood cells CD34 from a HLA-identic donor, which was tested for homozygosity with the CCR5 delta32 allele. The transfer was successfully completed. From the moment of transplantation, the patient does not require the use of therapy. Conclusion. Despite the success of the operation, this method of treatment is still difficult to implement, since transplantation is performed only in the presence of malignant hematological diseases, high histocompatibility of donor and recipient cells is required.
